This questionnaire is required before beginning Risen’s 2026 S.O.M. class. Risen Foundational Ministry Class starts April 12, 2026 and will end May 17, 2026. Classes will be held every Sunday 3-5pm. *There will be no class on May 10th due to Mother’s Day. *May 24th is individual meeting with the Pastors (Reserve your time slot on the last day of class). Ordination service will be June 7, 2026 at 3pm. The S.O.M. is available only to members of Risen Church.
